Abstract

A compact, hands-free, oral positive expiratory pressure (PEP) device designed to optimize respiratory mechanics by providing controlled resistance during exhalation. The device slows the respiratory rate, prolongs exhalation, generates positive airway pressure, and thereby reduces breathlessness, enhances oxygenation, and expands functional lung capacity. It is particularly beneficial in conditions such as chronic obstructive pulmonary disease (COPD), tracheobronchomalacia, excessive dynamic airway collapse (EDAC), bronchiectasis, atelectasis, and acute respiratory distress syndromes, while also serving broader functions like stress reduction, cardiovascular support, and respiratory muscle training. The device is lightweight, portable, and lanyard-supported, allowing continuous availability for daily use and physical activity. Structurally, it is conical, features a primary airflow chamber, a mouth opening, and one or more resistance orifices that regulate exhalation pressure. By maintaining consistent PEP levels (ranging approximately 4-15 cm H₂O), the device helps prevent airway collapse, promotes alveolar recruitment, and improves the ventilation-perfusion ratio.
