Abstract

The invention relates to a system for managing diabetes care for a patient. The system includes a data acquisition module configured to collect diabetes health data, such as glucose monitoring data, electronic health records, patient-reported diabetes data, and fitness data. A score-target interval mapping module calculates health dimension scores for glucose control, cardiovascular health, and diabetes resilience and maps each score to a corresponding health target interval using dimension-specific mapping functions. An appointment management module calculates target dates based on the elapsed time since the most recent appointment and the health target intervals. The module determines appointment parameters, including a primary target date based on the triggering health dimension with the earliest target date and assigns a healthcare provider role for the follow-up care appointment.
