A vaginal manipulator (20) includes a shaft (22) attached to a head (26) and a door (28) connected to the head. The head has an anterior side (40) opposite a posterior side (42) and includes a core (44) fixed to the shaft and a sleeve (46) coupled in a sliding relationship to the core. The door is coupled to the core and is movable relative to the posterior side of the head through an opening provided in the sleeve. A proximal end (48) of the core defines a proximal end of the head and a distal end (50) of the sleeve defines a distal end of the head. The proximal end of the core is fixed to the shaft. The sleeve moves relative to the core to provide the head with an adjustable length between the proximal end of the head and the distal end of the head. |

Disclosed is an artificial urinary sphincter and a kit of parts for providing urinary continence. The artificial urinary sphincter includes a casing, a coaptation valve including an iris diaphragm and a movable element. The iris diaphragm has an adjustable diameter and is movable to coapt a patient's urethra by moving the movable element. The movable element is associated with the casing and sized to be palpated through the perineal skin in order to adjust the diameter of the iris diaphragm. Also disclosed is a method of providing urinary voiding control in a patient. |

The head has a recess (30) formed in a first end surface of the head and two pads (32,34). The recess is sized to receive an inferior portion of the urethra. A first pad is located on a first side of the recess, and a second pad is located on a second side of the recess and spaced a gap distance away from the first pad. A piercing mechanism (62) is located in and movable out of one of the first pad or the second pad. The piercing mechanism is movable across the gap distance between the first pad and the second pad to form a channel in tissue superior to the urethra. |
