Custom patient-specific interbody implants, systems, and methods of generating custom implants. The implants may include upper and lower surfaces with an irregular surface topography, which are custom contoured to match the adjacent vertebral endplates of a specific patient. The method may include uploading images of the spine for the patient and processing the images into a virtual three-dimensional model of the spine including topography of individual vertebral bodies. The user may automatically and/or manually manipulate the virtual three-dimensional model to pre-operatively simulate a desired correction to the spine. The virtual implant model may be generated based on the desired correction and the topography of adjacent vertebral bodies.

Functional alignment of a 3D image of pelvis includes identifying a reference points on each 2D image of weight-bearing pelvis; identifying corresponding reference points on a 3D image of pelvis in a non-weight bearing position; constructing references on the 2D images based on the reference points on each respective image, constructing a corresponding reference on the 3D image based on the reference points thereon; and aligning the corresponding references of the 3D image with the references of 2D images, to provide a functionally aligned 3D image to more accurately plan placement of an acetabular shell. An independent, surgical screw placement system makes use of an aiming jig. The aiming jig has components and features which can be suitably adjusted so that an independent or extra-plate screw avoids crossing the nominal trajectories of other joint screws securing a surgical plate to a foot in an associated surgery. The aiming jig has a telescoping arm which can be adjusted to a variety of potential trajectories for the independent surgical screw until a desired trajectory for such independent screw has been determined.

In some cases, the surgical robot includes: a robot arm for holding a surgical instrument; and a controller coupled with the robot arm, the controller programmed to: detect a position of the surgical instrument in at least one attraction volume corresponding with a planned trajectory of the surgical instrument for the THA procedure, the at least one attraction volume defined in part by a point and an axis extending from the point; and while the surgical instrument is in the at least one attraction volume during the THA procedure, provide assistance to movement of the surgical instrument in a direction toward the at least one attraction volume.
