A coupling device for coupling a rod (100) to a bone anchoring element (1) having a head (3) and a shank (2) comprises a receiving part (5) having a first end (5a), a second end (5b) and a passage (51) extending from the first end (5a) to the second end (5b), a recess (54) for receiving the rod (100), the recess (54) extending from the first end (5a) towards the second end (5b) and forming two free legs (55) and an accommodation space (53) for accommodating the head (3) of the bone anchoring element (1), the accommodation space (53) having an opening (52) at the second end (5b), a pressure element (6) configured to be located at least partially in the accommodation space (53) and comprising a first portion (61) comprising a rod support surface (62a) and a second portion (66) having a hollow interior (67) with an opening defining a flexible portion configured to accommodate and clamp the head (3), wherein the receiving part (5) comprises a first securing portion (58a) and the pressure element (6) comprises a second securing portion (63a) configured to engage the first securing portion (58a), wherein the pressure element (6) is insertable into the receiving part (5) from the second end (5b) in a first rotational orientation relative to the receiving part (5) in which the first securing portion and the second securing portion are out of engagement and wherein the pressure element (6) is rotatable in the receiving part (5) to assume a second rotational orientation with respect to the receiving part (5) in which the first securing portion (58a) and the second securing portion (63a) are engaged to prevent the pressure element (6) from moving towards the first end (5a).

A bone anchoring device comprises a bone anchoring element (1) having a shank (2) for anchoring to bone and a head (3), and a coupling device for coupling a rod (100) to the bone anchoring element. The coupling device comprises a receiving part (5) having a first end (5a), an opposite second end (5b), and a passage (51) extending between the first end and the second end, the passage defining a central axis (C). The receiving part comprises an accommodation space (53) at the second end for accommodating the head of the bone anchoring element; a pressure element (6) configured to be arranged in the passage of the receiving part and to exert pressure onto the head received within the accommodation space to clamp the head at a particular angular position relative to the receiving part. The pressure element comprises at least one opening (66) that provides access to the inserted head from a direction transverse to the central axis; and at least one restraining element (7) configured to be arranged in the passage of the receiving part. The restraining element is movable relative to the pressure element in an axial direction between a first position and a second position, wherein in the second position the restraining element contacts the inserted head through the opening of the pressure element to restrict pivoting of the inserted head relative to the receiving part to a single plane.

Provided is an instrument for use with a bone anchoring device (2), the bone anchoring device (2) comprising a bone anchoring element (4) and a receiving part (5) for coupling a rod (3) to the bone anchoring element (4). The instrument (1, 1', 1", 1000) comprises at least a tube (10, 30, 40, 1030) attachable to the bone anchoring device, the tube defining a longitudinal axis (L), and at least one alignment member (20, 50) that is configured to assist in attaching the tube to the bone anchoring device.

In the receiving part, the pressure member can assume a first position in which the pressure member engages an inner wall portion of the passage such that the flexible portion is compressed to clamp or lock an inserted head with a first locking force and a second position in which an inserted head is clamped or locked with a second locking force smaller than the first locking force or is freely pivotable and wherein the pressure member is movable from the first position towards the second position by engagement with the instrument.
